How to Send a Sample

Instructions to order a genetic test:

Selection of the type of sample for each genetic test (see table)
Use the corresponding application form (see table) signed by the physician
Include:
- Informed consent of the patient (and relatives) subjected to the study
- Informed consent for cytogenetic test
- Brief clinical description, family history and family genealogical tree.
Sample extraction and parking:
Identification of each tube. Samples must be adequately packed to prevent breakage and sudden changes in temperature. DO NOT FREEZE the samples
Schedule delivery between Monday and Thursday avoiding weekends and holidays
